1. SPONSOR

This promotion (“Sweepstakes”) is funded and offered by ALPHA Men’s Health LLC, in support of ALPHA Men’s Health Medical PLLC (collectively, the “Sponsor”).

Medical services are provided solely by licensed medical professionals. Testosterone Therapy Treatment is only eligible to those who medically qualify. Should an individual Win the 'sweepstakes that is not eligible for Testosterone Therapy by the Sponsor’s medical evalutaion for whatever reason, they would default to the alternative prize as detailed in Section 5.

2. ELIGIBILITY

The Sweepstakes is open only to individuals (men and women) who, at the time of entry:

  • Are twenty-four (24) years of age or older

  • Reside within a twenty-five (25) mile radius of
    ALPHA Men’s Health
    1835 E. Southlake Blvd, Ste 100
    Southlake, TX 76092

  • Are legally eligible to receive medical care in the State of Texas

Employees, independent contractors, and immediate family members of the Sponsor are not eligible to participate.

3. PROMOTION PERIOD

The Sweepstakes entry period begins upon public announcement (8:00 AM Central Time on December 12th, 2025) and ends at 11:59 PM Central Time on March 31, 2026 (“Promotion Period”).

The prize drawing and winner announcements will occur in April 2026.

4. HOW TO ENTER

Participants may enter one (1) time per person using any ONE (1) of the following methods:

A. Automatic Entry — Current Patients

All current ALPHA Men’s Health patients in good standing during the Promotion Period will be automatically entered.

B. Free Consultation Entry

Complete a free on-site consultation and testosterone level check at ALPHA Men’s Health during the Promotion Period.
No purchase is required.

C. Mail-In Entry (No Purchase Necessary)

Mail a handwritten entry including:

  • Full legal name

  • Phone number

  • Email address

  • Date of birth

Mail-in entries must be received by March 31, 2026.

5. PRIZE DETAILS

Six (6) winners will each receive up to twelve (12) months of Testosterone Replacement Therapy (TRT) provided in accordance with ALPHA Men’s Health clinical standards. Should a winner be medically ineligible for testosterone therapy treatment, credit for alternative products and services for the ARV detailed below will be awareded to that winner with an expiry use period of 12 months from the date of issue.

Approximate Retail Value (ARV): $2,700 per winner.

6. MEDICAL ELIGIBILITY & ALTERNATIVE PRIZE

All prizes are subject to medical evaluation and clinical appropriateness as determined solely by ALPHA Men’s Health providers.

If a selected winner is deemed medically ineligible for TRT, the winner will instead receive a non-transferable credit of up to $2,700, redeemable toward eligible ALPHA Men’s Health products and services. IF a winner is deemed eligible at point of award and commences treatment, and subsequently becomes ineligible for continuation of TRT during the period through the assessment of ALPHA Men’s Health Medical Providers, then a credit will be issued for the balance of services not rendered. If a winner is unable to continue treatment due to (example: Relocation out of area of treatment jurisdiction (Texas), no credit or alternative will be issued.

No cash alternative or substitutions will be provided.

7. WINNER SELECTION & NOTIFICATION

Winners will be selected at random from all eligible entries.

Winners will be notified in April 2026 using the contact information provided at entry.

Failure to respond, complete required documentation, or meet eligibility requirements may result in forfeiture and selection of an alternate winner.

8. ODDS

Odds of winning depend on the total number of eligible entries received during the Promotion Period.

9. GENERAL CONDITIONS

  • Prize is non-transferable

  • Void where prohibited by law

  • Sponsor reserves the right to verify eligibility

  • Sponsor’s decisions are final and binding
